1. Purpose of Agreement
The purpose of this Agreement is to outline the terms and conditions under which [Contractor's
Company Name] (hereinafter referred to as the "Contractor") will provide services for the repair
or replacement of the roof at the property located at [Property Address] (hereinafter referred to as
the "Property") in relation to an insurance claim.
2. Insurance Claim
The Contractor has initiated an insurance claim for the repair or replacement of the roof. The
Homeowner agrees to allow the Contractor to work with the insurance company on the
Homeowner's behalf to facilitate the claim process.
3. Exclusive Service Provision
If the insurance claim is approved, the Homeowner agrees that the Contractor will be the sole
provider of the roof repair or replacement services. The Homeowner will not engage any other
contractor for these services.
4. Cost and Payment
a. The total cost for the roof repair or replacement will be outlined in a separate contract that will
be provided to the Homeowner once the insurance claim is processed and approved. This
subsequent contract will detail the final cost, which both parties must sign to formalize the
agreement.
b. We will work with your insurance company for up to one year to recover a fair amount of money to cover the payment not received for the roof repair or replacement. If we are unable to recover sufficient payment from the insurance, this amendment grants us the authority to act on your behalf, including legal representation, to pursue and receive all outstanding payments.
5. Contractor’s AuthorizationThe Homeowner grants the Contractor permission to act on their behalf and communicate with
the insurance company regarding the claim. This includes, but is not limited to, discussing claim
details, providing necessary documentation, and facilitating the claim approval process.
6. General Terms
a. This Agreement constitutes the entire understanding between the parties and supersedes all
prior agreements or understandings, whether written or oral.
b. Any amendments or modifications to this Agreement must be made in writing and signed by
both parties.
c. If any part of this Agreement is found to be invalid or unenforceable, the remaining provisions
shall remain in full force and effect, and the invalid or unenforceable provision shall be modified
to reflect the parties' original intent as closely as possible.
d. This Agreement shall be governed by and construed in accordance with the laws of the State
of Ohio.

By signing below, both parties acknowledge and agree to the terms and conditions set forth in
this Agreement.
